shadowdaoandClaude Sonnet 5.5 962d708fdd Rename Go module paths to repo.anhonesthost.net/CyberCoveLLC/threshold 
Rename root, api and sdk module paths (imports, go.mod, go_package,
ldflags, build scripts, buf configs) and regenerate protobuf/gateway
code with the project's pinned toolchain. Wire identifiers unchanged.
api gets a local replace for sdk; root/api/sdk require v0.1.0.

Jeff Mitchell 62d4f8453b Don't explode grants from the DB (#5104) 
This removes the Cartesian product in the DB for GrantsForUser in favor
of returning the actual grant scope information and dealing with it in
application code.

We had dupes and missing test cases, this fixes them.

* internal/handlers: move listtoken parsing into helper

This significantly simplifies the parsing logic needed in each handler.

Johan Brandhorst-Satzkorn d4d856e712 internal/target: refactor for new pagination design 
The new design uses four separate service functions; List,
ListPage, ListRefresh and ListRefreshPage. The first two are
used during the initial pagination phase and the latter two
are used for any subsequent refresh phases.
