Ryan Derr 7040374b6f Postgres 15 -> 16 Migration Update (#5825) 
* ICU-17235 fix(sql): add explicit casting to resolve Postgres 16 domain constraint changes

This migration updates the `wt_alias` and `wt_target_alias` domains to include explicit `::text` casting in their constraints. Postgres 16 enforces stricter domain checks, requiring explicit casts to ensure compatibility. These changes address upgrade issues by aligning the constraints with the stricter evaluation rules.

* ICU-16920 Refactor Boundary Database Init Flags for Improved Dependency Handling

This commit refactors the database initialization flags to prevent early exits and ensure explicit handling of flag dependencies. If one flag relies on resources skipped by another, the dependent flag will also be skipped.

Key Changes:
- Introduced a `InitFlags` struct to organize existing flags and establish a clear pattern for adding future flags.
- Added receiver functions on the `InitFlags` struct to determine whether a flag should be skipped based on provided options or dependencies.
- Refactored the `Run()` method to avoid early returns when a flag skips resource creation. Instead, a log message is provided to indicate the skipped option, and the process continues with resource creation.

* Fix #5602: Ensure controller status update triggers update_time

Rework and fix the logic of #5602 query split. Initial implementation failed to update controller status within `server_controller` as `w.Update` did not trigger `update_time` if `address` or `description` were not updated. Now using `w.Exec` with `updateController` query to always set `update_time` to `now()`.

Additional Notes:
- Updated `repository_controller` tests to verify `update_time` updates even when controller values don't change.
- Added `cleanupFunc` to test cases to remove inserted controllers after each test.
- Verified via `boundary dev` that `worker.upstreamDialerFunc` errors are not present.

At this stage we already know that at is non-nil, because
the case above would have been true if it was nil.

The previous error handling would have ignored API errors that were not ErrUnauthorized or ErrNotFound.
Handle all API errors the same as other unexpected errors.

These two error conditions are otherwise impossible

The default concurrency limit is 1, and it can be
set using the SetHashingPermits function.

Previously, the orphaned connection cleanup job would
not detect that a connection that was created before a
worker had submitted a successful session info was
orphaned. We now fall back to using the worker update
time as an indicator if the session info time is not set.

The new SessionInfo/RoutingInfo/Statistics RPCs run at
long intervals, which make some of our cluster tests take
a long time to complete. This allows their interval to be

This should speed up these tests significantly.

Timothy Messier 71af25f584 fix(db): Remove begin when assembling migrations (#5148) 
When building the set of migrations to run as part of executing the
`database migrate` or `database init`, the migrations are collected and
combined to run within a database transaction. As such the `begin` and
`commit` statements in the migration files should get removed from the
final set of statements that get executed. However, when the migrations
files were updated to include copyright and license headers, this broke
the logic that would strip out the `begin` statement.

While this does not cause any issue with executing the statements in a
single transaction, it does result in noise in the database server's
logs, with log messages like:

    BEGIN WARNING:  25001: there is already a transaction in progress
    BEGIN LOCATION:  BeginTransactionBlock, xact.c:3778

This commit fixes the logic to correctly strip the `begin` for files
that have the header lines.
